Re-send my requisition


Requesting a Requisition you have already been provided


Option #1:
If we have previously emailed you a requisition - or - you have lost the requisition that was given to you in clinic, you can request it re-sent by sending a message to our administrative team (if you have consented to receive medical emails from us). Please expect 3-5 for a response (it is unlikely we will be able to action urgent requests same-day).

OR

Option #2: If you would like us to fax an existing requisition to a facility, please send our administrative team a message, please be sure to include the fax number of the imaging facility you will be visiting. Please let us know at least 3-5 days in advance if possible (it is unlikely we will be able to action urgent requests for immediate faxes same-day).


Requesting a NEW requisition

Option #3: If you are requesting a new requisition (which likely does not already exist in your chart) please book an appointment with your physician.
