Overview
How do you register to be a patient?
As demand for family physicians continues to be extremely high throughout Ontario, we have begun to prioritize individuals who live in the communities that we most directly serve.
Step 1. Filling in the form below will let you know if we are currently accepting new patient registrations from your home postal code.
Step 2. If you are within our catchment area, we’ll ask for more information to validate your OHIP.
Step 3. Once this is done, we will invite you by email to schedule a new patient intake appointment with a particular doctor (a “meet and greet” appointment).
WHEN DO I BECOME A PATIENT?
Completing the forms is the first step to become a patient of one of the family physicians at Magenta Health; you do not automatically or immediately become a patient.
Instead, you’ll be invited to have an intake appointment with a particular doctor first. Only at the end of that appointment, should you decide to proceed, will you become a patient.

It varies depending on your preferred clinic location. You’ll be provided more specific and up-to-date information as part of completing the forms.

Do we have a catchment area?

can i have multiple family doctors?
No. If you currently have a family doctor, please register with us only if you intend to transfer your medical care from your current family physician (i.e. leave your current family physician).
can i meet with multiple doctors?
No. Kindly note that it is not an option to switch locations or physicians after you have had your new patient intake appointment.
I was previously a patient / I think I registered previously. should i register again?

CAN I REGISTER FOR MY PARTNER OR FAMILY?
Yes. Kindly complete this form in its entirety for each individual.
If you are hoping to share a physician with your family member, we can accommodate this in some circumstances. Please see the next page regarding these requests.
can I register just my child?
Our standard policy is that we do not accept infants or children under 5 years of age unless at least one parent or guardian is enrolled as a patient as well. Children under 5 must be registered to the same physician as their parents. Please also fill out the form for your child if you decide to start the registration process for yourself.
I just had a baby… what do i do?
If at least one parent is an active patient of a Magenta Health physician, please register your newborn here instead.
If that’s not the case, sorry - we recommend that you look for another clinic to see your newborn at this time. Due to overwhelming demand, we’re unable to accommodate you and your newborn in a timely manner. Our sincere apologies.
