Updating your contact information And/Or Email consent


To update any ONE piece of information (e.g. address / phone / email / email consent), use this form.

To update TWO OR MORE pieces of contact information, or if you no longer have access to the email on file, follow these instructions.

If you have just turned 18 and previously had a guardian’s email on file, you can follow the instructions here to update our records to your own email address.


Updating your personal information

Generally, our records have to match what is written on your OHIP card. This is in part to minimize the risk of medical errors either by our clinic, or other third parties. For this reason, we regularly audit our database against the Ministry of Health, and any changes to your name as it appears on your OHIP card will automatically be updated within a few months of the change.

To respect and celebrate our patient’s personal identities, we have also added a spot to our records to indicate the patient's name that they go by,  if it is different from their name as it appears on their OHIP card. If you would like to update or notify us of the name you go by, please send our admin team a message.
